All-cause mortality versus staying inactive
The short version
- •The steepest part of the curve is at the bottom: going from nothing to just 15 minutes of daily activity was associated with 14% lower mortality and about 3 extra years in a cohort of 416,000 (Lancet, 2011).
- •The sweet spot is 150 to 300 minutes a week. Pooling 661,000 adults, people at 1 to 2 times the guideline minimum had 31% lower mortality, benefits leveled off around 3 to 5 times, and even 10x showed no harm (JAMA Internal Medicine, 2015).
- •Fitness beats every classic risk factor: in 750,000 veterans, low cardiorespiratory fitness predicted death more strongly than smoking, diabetes, or hypertension (JACC, 2022).
- •Strength is its own lever: 30 to 60 weekly minutes of resistance work is associated with 10 to 17% lower all-cause mortality, and grip strength out-predicted systolic blood pressure for death in the PURE study.
- •Timing barely matters: weekend warriors who cram the same weekly dose into 1 or 2 sessions get essentially the same mortality benefit as people who spread it out (JAMA Internal Medicine, 2017).
- •Starting in middle age still works: people who became active saw 24 to 29% lower mortality regardless of their past (BMJ, 2019).
Exercise is the closest thing longevity research has to a wonder drug, and the dose-response curve has been mapped in cohorts of hundreds of thousands. The shape of that curve is the most useful fact in this article: it is steepest at the very bottom, which means the least fit have the most to gain from the smallest change. Here is what matters most, ranked by the strength of the evidence, each with one thing you can actually do.
Any movement at all
The jump from nothing to a little is the biggest on the whole curve. In 416,000 Taiwanese adults, 15 minutes of daily moderate activity was associated with 14% lower all-cause mortality and about 3 extra years of life versus being inactive (Wen, Lancet 2011). Every additional 15 minutes cut mortality another 4%.
Do this
If you currently do nothing, do not design a program, just walk 15 minutes a day starting today. You capture the single largest slice of the benefit with the first slice of effort.
150 to 300 minutes a week
Pooling 661,000 adults across 6 cohorts, meeting 1 to 2 times the guideline minimum of 150 weekly minutes meant 31% lower mortality, with the optimum around 3 to 5 times the minimum and no harm detected even at 10 times (Arem, JAMA Internal Medicine 2015).
Do this
Aim for 30 to 45 minutes of something brisk most days. Past roughly an hour a day the longevity return flattens, so more than that is for sport, not span.
Getting actually fit, not just active
Cardiorespiratory fitness is the strongest single mortality predictor ever measured: in 750,000 US veterans, being unfit out-predicted smoking, diabetes, and hypertension (Kokkinos, JACC 2022), and in 122,000 treadmill-tested adults the fittest had about 80% lower mortality with no upper limit found (Mandsager, JAMA Network Open 2018).
Do this
Once you move regularly, add intensity: 2 sessions a week hard enough that talking is difficult, for example 4 rounds of 4 minutes hard with 3 easy. Intervals raise VO2max about 2.5x faster than steady cardio.
Strength, twice a week
Independent of cardio, 30 to 60 weekly minutes of resistance training is associated with 10 to 17% lower all-cause mortality (Momma, British Journal of Sports Medicine 2022), and in the 140,000-person PURE study grip strength predicted death better than systolic blood pressure (Leong, Lancet 2015). Muscle is also the organ that buffers blood sugar and survives falls.
Do this
Two short full-body sessions a week: push, pull, squat, carry. Bodyweight at home counts; the sessions you actually do beat the program you skip.
About 7,000 steps a day
Steps are the floor under everything else: compared with 2,000 a day, about 7,000 is associated with 47% lower all-cause mortality, with only small additional gains beyond 10,000 (Lancet Public Health 2025 meta-analysis of 57 studies).
Do this
Walk every phone call, park far on purpose, and take a 10 minute walk after your biggest meal, which also flattens the glucose spike.
Breaking up the sitting
Long uninterrupted sitting tracks with higher mortality, but it is not destiny: in a harmonized meta-analysis of over 1 million people, 60 to 75 minutes of daily moderate activity essentially eliminated the extra mortality associated with 8 or more sitting hours (Ekelund, Lancet 2016). Less active people did not get that protection.
Do this
Stand or walk 2 to 3 minutes every hour of desk time, and treat your daily activity minutes as the antidote your chair makes necessary.
The dose, not the schedule
Weekend warriors who compress the full weekly dose into 1 or 2 sessions show essentially the same reductions in all-cause, cardiovascular, and cancer mortality as people training 5 days a week (O’Donovan, JAMA Internal Medicine 2017). The body banks total weekly work.
Do this
Stop waiting for a week with free evenings. If Saturday and Sunday are all you have, a long session each is a legitimate program, not a compromise.
Starting late still pays
In 14,500 UK adults followed across middle age, those who increased activity over the years had 24 to 29% lower all-cause mortality, and the benefit held regardless of how inactive they had been before, and even in people with existing heart disease or cancer (Mok, BMJ 2019).

If you are 50, 60, or 70 and have never trained, the curve still works for you. Start at item 1 like everyone else, and climb.
